So I've been in my tribe house doing a test to get ranked and this map had death coins on it.I never touched the death coin and I died.

My theory, which might not be right but I don't know what else it could be is:
Lag/ping probably. The next time that happens type /ping into your chat box. If it's over let's say, 600 then that's probably it.
When you lag weird things happen and sometimes you think your mouse is somewhere but it's actually in another spot on the map.

My ping was 171.
