This seems to be a recent problem. Some players have been dying for no apparent reason when they enter the mouse hole with cheese, or have Easy Victory used on them, regardless of the time remaining. Has anyone else noticed this?

Yes, it's a known issue but unfortunately there's no resolution at this time.
