My crown molar. I had a metal spacer at the time and I had always fidgeted with it until it broke, but when it did my crown popped off with it. I continued to mess with my crown cause it would lift off my tooth, but I was chewing gum one day and it got stuff on the crown and I had choked on it. I ended up swallowing it.

One of those candy life-saver rings.
It got lodged in my throat for about 2 class periods, and needless to say I honestly would have choked to death without the hole in the middle of the candy.
