It's kind of hidden. There's a guy with a quest and a bridge next to him. One of his quests involves saving a little girl and SHE gives you the task to beat the boss.

You don't get the title by killing the boss, you have to complete all of the quests on Siwa to get it, but in one of those quests, you do kill it.
