Well you can't insert images into maps, but you can do that with LUA.

I don't know too much about LUA, so try asking here. Link

They never even existed, how are they to be brought back?

Not to mention, map crew truly has no control over this. It would be up to admins. Map crew just looks at maps and deems them worthy for higher rotation.

They did exist, just not as a lone ground.

They never existed as a proper ground. Some people used colourjoints, 0,0 glitch and defilante's offscreen killing mechanic to simulate what it could look like.

What is temporary perm?

A map may be voted to only be perm for a month for various reasons. We'll come back to it to discuss it further after playing it a few times in public during the month.

Are there any list of temporary perm maps? How can i know my (anybody) map is temporary perm or not?

I've had at least 1, and I know many other maps of mine have been considered (but denied the month long perm anyway) so I've had some experience with this. You'll know about it, in the submission threads they will say if they'll consider giving it a month long perm, and generally if you have a map in discussion, or being considered for discussion, staying in touch with the Mapcrew about it will let you know what all is going on, they won't share secret info with you, but they do tell you if your map is being discussed, just like in the submitting threads.
