I have had this issue too. Wait for 3-6 days and your picture in tfm will be updated.

That highly depends on the situation, context and length of your ban. I recommend you take it up privately with any moderator (so yeah you can also pm me I'll be happy to look into it).

You have to earn the said title over again. For example with Nice Mouse, where you have to buy 1 item, you'd first have to earn the rest of the titles before earning them again.
